Before you receive your Carte Vitale, when you consult a doctor or go to a pharmacy to buy prescribed medication, the healthcare professional will give you a document called a "feuille de soins" (treatment form), which serves as proof of healthcare expenses to request reimbursement from the French health insurance (Assurance Maladie).
Once the "feuille de soins" has been duly completed, simply send it by mail to the relevant primary health insurance fund (CPAM):

⚠️ Important
- If you have not yet received your social security number (even a temporary one), keep your treatment forms carefully. You can send them later, once you are able to fill in the dedicated field for the number.
- If you have a temporary number, it can be used on the treatment form while waiting for the definitive number.
- If you have forgotten your Carte Vitale or if the healthcare professional is not equipped for teletransmission, a treatment form may also be given to you.
- This document only exists in French.
